Brigham and Women's Hospital offers a variety of resources to help both new and established patients prepare for a visit to our practice.

If you are a new patient, please bring the following to your appointment:

• Copies of Imaging CDs/Films, Related Test Results, and Office Notes Completed at Outside Facilities
• Insurance Card and Referral
• Co-payment
• Completed New Patient Intake Form
• Patient Safety Brochure

If this is a follow-up visit, please bring the following to your appointment:

• Insurance Card and Referral
• Co-payment
